Bite Adjustments for Underbites

A bite adjustment is often recommended for patients who experience discomfort, difficulty chewing, or aesthetic concerns due to an underbite. This treatment supports long-term jaw health and function by carefully aligning the teeth for better balance. An orthodontist customizes the process based on tooth shape, jaw structure, and bite pressure to create a more natural, comfortable fit.

What is an underbite?

An underbite occurs when the lower teeth extend farther out than the upper teeth. Orthodontists often classify malocclusions (misalignments) into a class system—class one, two, and three. According to the Cleveland Clinic, underbites fall under class three, as they impact the lower jaw and teeth. They can affect chewing, speaking, and facial symmetry. In some cases, an underbite may lead to abnormal wear on the teeth or jaw joint pain. Addressing the issue early can help prevent additional complications over time.

The role of professional bite adjustment

Bite adjustment from an orthodontist plays an important role in correcting underbites caused by uneven tooth contact. This procedure involves the subtle reshaping of specific tooth surfaces to improve alignment. With the help of oral appliances, bite adjustment helps to redistribute pressure evenly across the bite. The result is:

  • Reduce the risk of chipped or worn teeth
  • Relieve tension in the temporomandibular joint (TMJ)
  • Improve bite force and chewing efficiency
  • Prevent uneven erosion of enamel

Appliances used for bite adjustment

As mentioned, appliances aid bite adjustments. However, the type of tool or appliance an orthodontist employs will depend on the severity of the underbite. For example, a mild to moderate underbite can benefit from clear aligners. Clear aligners are virtually invisible, removable trays that fit snugly over the teeth. Over time, they gradually shift the teeth into better positions. However, patients must wear the aligner trays for a minimum of 22 hours daily, and treatment can last six to 18 months.

Braces are another tried-and-true option that orthodontists rely on. They can correct mild to severe forms of underbites. Unlike the removable clear aligners, braces use brackets and wires that the orthodontist bonds to the front surface of the teeth. Over time, the orthodontist will tighten the wire as the teeth and jaw adjust into a more desirable position. Treatments can span anywhere from six months to three years. For underbites too severe for an appliance to correct, the orthodontist may recommend the patient to an oral surgeon for surgical bite adjustment, but this often requires braces afterward as well.

Whether a bite adjustment plan involves clear aligners or braces, the orthodontist will fit patients with a retainer after completing active treatment. This oral appliance is often clear in color and holds the teeth in their new positions. Some patients may wear them all day, while others may only need them at night; either way, the orthodontist will determine the best course of action to prevent the underbite from returning.
